This form was created for digital navigators to get a general idea of a community member’s comfort with technology and to measure any change in their comfort over the course of their work with the digital navigator program.
It is best practice for a digital navigator to work through this form conversationally to learn more about the individual and begin establishing a relationship during their first meeting. Digital navigators may repeat this assessment throughout their time working with a community member at a cadence that fits their program. It may be repeated at the end of their expected final meeting with a digital navigator, which may be their first and only meeting, or on a schedule such as every fourth meeting. Some programs only use the form with community members who are seeking digital skills support and do not ask community members to these questions if they are only seeking device or connectivity support.
